questionnaire
A document that contains a structured series of questions. Questionnaires are often used in auditing and *internal control survey. In particular, *Control Risk Self Assessment techniques are frequently based on questionnaires. The limitations of questionnaires have often been emphasized by critics: for example, they have been called "subjective and easy to manipulate" (Toffler and Reingold, 2003, 144). However, they offer the advantage of being a cost-effective means of obtaining information in large organizations. See also *checklist and *box-ticking approach.
